Individual Forms

Parental Permission/Medical Release

All players must have a signed medical release form on file with the school so that proper medical care can be given to an injured played when the injured player's parents/guardians cannot be contacted. All medical release forms are kept on site by all coaches at all times.

Athlete Emergency Information

This required form covers emergency contact information, health information, and insurance information.

Physical Examination

St. J. A. requires a physical examination a minimum of every year in order to participate in interscholastic athletics. Physical exams are at the expense of the individual, and a record of the physical must be on file in the Athletic Director’s office before a student is eligible to practice or compete. If the one year time limit expires during the season, the student will be denied participation until his/her physical is renewed. All players must have a current sports physical form on file with Mr. McGinn before they will be allowed to participate in practices or games. A physician's form, clearing an athlete for participation, can be faxed to the SJA Athletic Department at 751-2117. Physicians forms must be on the doctor's letterhead.

Athletic Commitment

At St. Johnsbury Academy, students may choose to participate in a wide selection of sports. Participation requires the student and family to commit to the team for the entire season. “Your attendance at all practices and games is required unless you are properly excused.” (Student Handbook) Practices and scheduled events run Monday thru Saturday and include school breaks, vacations, and for spring sport athletes, being on campus after graduation. All athletes are required to sign the Athletic Commitment form, which outlines the attendance requirements for the Athletics program.
